Beginning July 15th, 2025, NYRx, the Medicaid Pharmacy Program, will accept electronic PA requests via CoverMyMeds®. Prescribers and authorized agents: register for an ePA webinar or reach out to NYRxEO@primetherapeutics.com for more information.

Prior Authorization Resources
To obtain a prior authorization call (877) 309-9493 or fax the appropriate Prior Authorization form to (800) 268-2990. The clinical call center is available 24 hours a day, 7 days per week.
PAXpress is a web-based application available for initiating prior authorization requests.
